Bu Zhong Yi Qi Tang: The Ultimate Formula for Chronic Fatigue

Updated March 2026 ยท 8 min read

Feeling exhausted no matter how much you sleep? Catching every cold that goes around? In Traditional Chinese Medicine, these are hallmarks of "sinking Qi" โ€” and Bu Zhong Yi Qi Tang has been the master formula for this pattern since the 13th century.

What Is Bu Zhong Yi Qi Tang?

Bu Zhong Yi Qi Tang (่กฅไธญ็›Šๆฐ”ๆฑค), meaning "Tonify the Middle and Augment the Qi Decoction," was created by the legendary physician Li Dongyuan (ๆŽไธœๅžฃ, 1180โ€“1251 CE) in his groundbreaking text Pi Wei Lun (่„พ่ƒƒ่ฎบ, Treatise on the Spleen and Stomach).

Li Dongyuan's revolutionary insight was that many chronic diseases stem from damage to the Spleen and Stomach โ€” the body's digestive engine. When the Spleen Qi becomes so weak that it "sinks," the body loses its ability to hold things up, both physically (organ prolapse) and energetically (chronic fatigue, depression).

Ingredients

  • Huang Qi (้ป„่Šช) โ€” Astragalus: The chief herb, powerfully tonifies Qi and raises Yang
  • Ren Shen (ไบบๅ‚) โ€” Ginseng: Strongly tonifies Spleen and Lung Qi
  • Bai Zhu (็™ฝๆœฏ) โ€” Atractylodes: Strengthens the Spleen, dries dampness
  • Zhi Gan Cao (็‚™็”˜่‰) โ€” Honey-fried Licorice: Tonifies Qi, harmonizes the formula
  • Dang Gui (ๅฝ“ๅฝ’) โ€” Angelica: Nourishes Blood (Qi and Blood are inseparable)
  • Chen Pi (้™ˆ็šฎ) โ€” Tangerine peel: Moves Qi, prevents stagnation from the tonics
  • Sheng Ma (ๅ‡้บป) โ€” Cimicifuga: Raises the Yang Qi upward
  • Chai Hu (ๆŸด่ƒก) โ€” Bupleurum: Raises the Yang Qi, spreads Liver Qi

The genius of this formula lies in Sheng Ma and Chai Hu โ€” two "raising" herbs that lift the sunken Qi back to its proper position. Without them, the tonifying herbs would just add energy without direction.

Who Needs Bu Zhong Yi Qi Tang?

1. Chronic Fatigue

Not ordinary tiredness, but deep exhaustion that doesn't improve with rest. The person feels worse after exertion, speaks in a weak voice, and may feel dizzy when standing up. This is the formula's primary indication.

2. Weak Immunity

People who catch every cold, take weeks to recover from minor illnesses, or get recurrent infections. In TCM, the Wei Qi (defensive Qi) depends on strong Spleen Qi. When the Spleen is weak, the body's defenses collapse.

3. Organ Prolapse

When Qi is too weak to hold organs in place: uterine prolapse, rectal prolapse, gastroptosis (stomach dropping), or chronic hemorrhoids. The "raising" herbs in this formula specifically address this sinking pattern.

4. Chronic Diarrhea

Long-term loose stools or diarrhea that worsens with fatigue or stress. The Spleen is too weak to transform food properly, and Qi is sinking downward instead of holding firm.

5. Post-Illness Recovery

After prolonged illness, surgery, or childbirth when the body's Qi reserves are depleted. Bu Zhong Yi Qi Tang rebuilds the foundation.

Dosage

  • Patent pills (่กฅไธญ็›Šๆฐ”ไธธ): 8-10 pills, 2-3 times daily
  • Granules: 1 packet, 2-3 times daily
  • Best time: 30 minutes before meals on an empty stomach
  • Duration: 2-8 weeks depending on severity

Bu Zhong Yi Qi Tang vs Gui Pi Tang

  • Bu Zhong Yi Qi Tang: For Qi sinking โ€” fatigue, prolapse, weak immunity. Focus is on RAISING Qi.
  • Gui Pi Tang: For Heart-Spleen deficiency โ€” insomnia, anxiety, poor memory. Focus is on NOURISHING Blood and calming the mind.

Both treat Spleen Qi deficiency, but from different angles.

Who Should Avoid It?

  • People with Yin deficiency and Heat signs (night sweats, hot flashes)
  • During acute infections with fever
  • People with high blood pressure (the raising herbs may worsen it)
  • Those with excess patterns (strong constitution, loud voice, red face)
